眼科病床的合理安排
摘 要
本文全面分析了医院病床安排所存在的问题,影响问题的因素,并利用病人满意度和医院工作效率作为评价指标,对现有的FCFS模型进行了优劣分析,建立了更为高效的综合病床安排模型。
问题一:为了能够更加合理的对医院病床使用情况做出评价,建立了病人满意度MYD,床位使用率,床位周转次数N0,病床使用指数评价体系。求出病人满意度MYD4.909,床位使用效率1,床位周转次数N040.7027,病床使用指数1.2719。
问题二: 医院在安排病人入院时要考虑多方面的因素,不仅要考虑病人的满意度,而且还要使医院资源的最大化利用。在安排已入院的病人手术时,给不同眼科疾病的病人的入院顺序赋予不同的优先级,根据每天的空病床数,决定入院病人的名额,在依次选取的一组未入院的门诊病人中按照优先级的高低依次安排人院,同种病人按门诊时间的先后顺序依次安排入院。
问题三:为了能够让病人在门诊时就能被告知大致入住医院的时间区间,我们根据当时已出院病人的数据,统计得到除急症病人外,其他病人的平均住院时间,然后建立正态分布的区间估计模型,对门诊病人的住院时间进行估计。计算可得,平均等待时间的置信度为10.95的置信区间为(12.5391,12.7595),即可认为病致入住医院的时间区间为(12,13)。
问题四:周六周日不安排手术,则星期四、五的安排手术的时间就会发生变化,因此对星期四、五的对未住院的不同疾病的门诊病人的优先级进行相应的调整可得到到最优值。
问题五:为了使得所有病人在系统内的平均逗留时间T最短,以T做为目标函数,建立线性规划模型,用LINGO软件求解得到各类病人占用病床的比例:

1.问题重述
医院就医排队是大家都非常熟悉的现象,它以这样或那样的形式出现在我们面前,例如,患者到门诊就诊、到收费处划价、到药房取药、到注射室打针、等待住院等,往往需要排队等待接受某种服务。
我们考虑某医院眼科病床的合理安排的数学建模问题。
该医院眼科门诊每天开放,住院部共有病床79张。该医院眼科手术主要分四大类:白内障、视网膜疾病、青光眼和外伤。附录中给出了2008年7月13日至2008年9月11日这段时间里各类病人的情况。
白内障手术较简单,而且没有急症。目前该院是每周一、三做白内障手术,此类病人的术前准备时间只需1、2天。做两只眼的病人比做一只眼的要多一些,大约占到60%。如果要做双眼是周一先做一只,周三再做另一只。
外伤疾病通常属于急症,病床有空时立即安排住院,住院后第二天便会安排手术。
其他眼科疾病比较复杂,有各种不同情况,但大致住院以后2-3天内就可以接受手术,主要是术后的观察时间较长。这类疾病手术时间可根据需要安排,一般不安排在周一、周三。由于急症数量较少,建模时这些眼科疾病可不考虑急症。
该医院眼科手术条件比较充分,在考虑病床安排时可不考虑手术条件的,但考虑到手术医生的安排问题,通常情况下白内障手术与其他眼科手术(急症除外)不安排在同一天做。当前该住院部对全体非急症病人是按照FCFS(First come, First serve)规则安排住院,但等待住院病人队列却越来越长,医院方面希望你们能通过数学建模来帮助解决该住院部的病床合理安排问题,以提高对医院资源的有效利用。
问题一:试分析确定合理的评价指标体系,用以评价该问题的病床安排模型的优劣。
问题二:试就该住院部当前的情况,建立合理的病床安排模型,以根据已知的第二天拟出院病人数来确定第二天应该安排哪些病人住院。并对你们的模型利用问题一中的指标体系作出评价。
问题三:作为病人,自然希望尽早知道自己大约何时能住院。能否根据当时住院病人及等待住院病人的统计情况,在病人门诊时即告知其大致入住时间区
间。
问题四:若该住院部周六、周日不安排手术,请你们重新回答问题二,医院的手术时间安排是否应作出相应调整?
问题五:有人从便于管理的角度提出建议,在一般情形下,医院病床安排可采取使各类病人占用病床的比例大致固定的方案,试就此方案,建立使得所有病人在系统内的平均逗留时间(含等待入院及住院时间)最短的病床比例分配模型。
2.模型假设及符号说明
2.1.模型假设
(1)由于视网膜疾病和青光眼急症数量较少,而白内障没有急症,在建模过程中这些眼科疾病我们不予考虑急症情况。
(2)该医院手术条件比较充分,在考虑病床安排时可不考虑手术条件的。 (3)每个病人只患一种病,不考虑一个病人兼患两种病(或以上)的情况。 (4)假设样本数据有代表性,能够反映医院眼科门诊每天接收各类病人的数量的分布以及各类病人术后的观察时间的分布。
(5)这个问题中,在FCFS条件下,为减少排队人数,假设医院的床位被全部利用,即医院的病床使用率始终为100%。
(6)病人门诊当天不能入院,最快要第二天入院。 2.2.符号说明

3.模型的分析、建立及求解
3.1.模型一:评价指标体系模型
我们首先给出一个总的评价指标体系模型,对医院现行的病床安排以及我们以后给出的病床合理安排模型进行评价。 3.1.1.模型分析
对医院的病床安排问题,一个合理的评价指标体系必须要包括以下2个方面的因素:
(1)病人的满意度。病人从到医院门诊时开始接受医院的服务,从门诊到入院的等待时间和从入院到接受手术直至出院的等待时间构成了病人对医院的满意程度。一般来说,等待时间越长满意度越低,最后趋于平缓。而且从入院到手术的等待时间t'ij比从门诊到入院的等待时间tij的影响要更大一些,因为病人在入院了之后要缴纳住院费。
(2)医院工作效率。对医院方面来说,良好的工作效率构成了评价一个医院指标好与坏的前提,其主要包括两个方面:床位使用率和病人周转率。 3.1.2.模型建立
(1)病人的满意度
病人的满意度跟病人从门诊到入院的等待时间tij和从入院到手术的等待时间t'ij有关。一般来说,从入院到手术的等待时间t'ij比从门诊到入院的等待时间tij对病人满意度的影响更大一些,因为病人从入院到手术的等待时间内要缴纳手续术。我们对病人从门诊到入院的等待时间tij取平均值tij,对病人从入院到手术的等待时间取平均值t'ij,因为t'ij比tij对病人的满意度的影响更大一些,我们对tij取权值w=0.3,对t'ij取权值w'=0.7,得病人的满意度评价指标如下:

由上面的分析知,病人的满意度对眼科病床的安排有明显的影响,MYD越小,则说明眼科病床的安排越合理;越大,则说明眼科病床的安排不太合理,还有待改进。
(2)医院工作效率
床位使用率:病床使用率=实际占用总床日数d/实际开放总床日数D,查阅国家标准,≥60%。它反映平均每天使用床位与实有床位的比例情况。使用率高,表示病床得到充分使用;反之,则说明病床空闲较多。
床位周转次数:床位周转次数n0=周期内入住人数(或者退出人数)NT/床位数N,查阅国家标准,医院年病床周转次数≥32。床位周转次数是衡量医院床位周转速度快慢的指标。平均床位工作日和床位使用率,只能说明床位的一般工作负荷状况,还不能表示床位的工作效率情况。分析病床工作效率时,应注意在一定时间内,周转次数多即周转速度快、病床利用情况好,病人平均住院天数少。
病床使用指数:病床使用指数=病床周转次数n0/标准周转次数N0*病床使用率。
综合上述的评价模型,我们可以得出一个合理的评价方案:

n0/N0对上面的分析知,病床使用指数反映了一个医院的工作效率,若病床使用指数越大,则医院的工作效率越高;若病床使用指数越小,则医院的工作效率越低。
3.1.3.模型求解
我们对以上建立的模型进行求解,可以求得病人的满意度和医院的工作效率指标。
3.1.3.1.病人满意度
通过病人满意度模型的建立,MYDwtijw't'ij;,我们利用Matlab对所给出的数据进行处理,得出病人的满意度MYD4.909。
3.1.3.2.医院工作效率
8月8号到8月28号医院病床使用数量达到动态平衡的饱和状态,见下表:

3.2.1.问题分析及模型建立求解
医院在安排病人入院时要考虑多方面的因素,不仅要考虑病人的满意度,而且还要使医院资源的最大化利用。在安排已入院的病人手术时,考虑白内障手术较简单,而且没有急症,此类病人的术前准备时间只需1、2天。目前该院是每周一、三做白内障手术,如果要做双眼是周一先做一只,周三再做另一只。可是在我们对该医院的病人数据进行处理后,发现白内障和白内障(双眼)病人入院后等待手术的时间分别为2.333和3.579天,与正常的术前准备时间不否,多出的准备时间会给病人增加多余的经济负担,使病人满意度降低,同时造成了医院的资源浪费。
通过对问题的分析,在安排病人入院时,针对目前该医院所存在的资源浪费问题以及减少每种病人的术前准备的时间,我们将每天的未人院的门诊病人作为一组,一起作为考虑人院的对象。
考虑到病人的术前准备时间以及对患白内障疾病病人的手术时间的特殊安排.在安排入院时,对一星期中的七天。每天给不同眼科疾病的病人的入院顺序赋予不同的优先级,根据每天的空病床数,决定人院病人的名额,在依次选取的一组未入院的门诊病人中按照优先级的高低依次安排人院,同种病人按门诊时间的先后顺序依次安排入院。即患有不同疾病的病人可以按优先级在有空床的当天入院,相同疾病的病人按照FCFS规则选择入院。为实现该模型,经过对大量数据的分析和手术安排的规划,规定一星期七天中每天对不同疾病未入院的门诊病人的优先级(4>3>2>1),其具体级别如表1所示。

Step1:估计表二中病人的出院时间,选出表二中最早出院时间,并查找出在此
天出院的人数及空床数,判断此天是星期几,然后根据上述优先级表格安排此天住院人;
Step2:根据上述安排估计出被安排住院的人的出院时间,将已安排住院的那天
日期记为无穷大(即再选出院时间就不会在选这一天,而选这天之后的);
Step3:重复以上步骤,直到表三中门诊病人都被安排住院。

3.2.2.模型建立的检验
本文在问题二中建立的优化模型后,预测出题目中表2、表3的数据,包括入院时间,手术时间,出院时间,按照问题一给定的满意度的指标进行评价,检验,最终得到综合满意度为MYD4.424,与第一问的MYD4.907相比显得更优秀,验证了模型的合理性。
针对问题二中排队模型的Matlab代码见附录6。 3.3.模型三:区间估计模型
为了能够让病人在门诊时就能被告知大致人住医院的时间区间,我们根据当时已经出院病人的数据,统计得到除急症病人外,其他病人的平均住院时间,然后建立区间估计模型,对门诊病人的住院时间进行估计。在不考虑外伤的情况下,可近似认为病人的服务时间即等待时间tij服从正态分布N(,2),其中方差2未知,故平均等待时间的置信度为10.95的置信区间为
(tijt(n1)21SnS,tijt(n1)n)1nn 2计算可得,置信区间为(12.5314,12.7672)。故可以在病人门诊时告知其大致入住院时间区间为(12,13),即住院时间大概是在门诊后的第12或13天。 3.4.模型四
3.4.1.模型建立及求解
周六周日不安排手术,则星期四、五的安排手术的时间就会发生变化,因此

通过对建立的模型进行仿真与分析后,认为该医院目前所采取的FCFS规则,虽然可以解决大部分病人的住院问题,但是不能够使医院的资源得以合理有效的利用,同时损害了病人的利益,使病人的满意率下降;而基于优先级的病床安排,不仅可以权衡病人和医院双方的利益,而且使排队系统的绝大多数的运营指标均处于最优状态.得到了最优化的处理结果。 3.5.模型五:线性规划模型 3.5.1.模型分析
从便于管理的角度出发,在一般情形下,医院病床安排可采取按照各类病人占用病床的比例进行分类排队的方案。 3.5.2.模型建立
因为单眼白内障和双眼白内障病人所需住院时间不一样,把病人再细分为外伤、单眼白内障、双眼白内障、青光眼和视网膜疾病五类。设这五类病人的占用病床比例为x1:x2:x3:x4:x5。病人在系统内的平均逗留时间T主要由等待入院时间axi、术前准备时间bxi和术后观察时间cxi决定。为了使得所有病人在系统内的平均逗留时间T最短,我们以T做为目标函数,建立线性规划模型:

(1)xi都不能超过79张床,且x1,x2,x3,x4,x5的和为79;
(2)根据医院历史的统计情况,对不同情况作出统计,五种病所占的比例约为:12,13,20,23, 9,所以需满足 x112,x213,x320,x49,x59;
(3)病床数xi为正整数,各种病床总数之和为79。
考虑不同病症的病情、手术时间安排和术后观察时间长度等情况各不相同,术前准备时间长和术后观察时间长的病症因为病床周转慢,所以应多分配占用多一些床数,统计2008-07-13到2008-09-11的病人信息表中可以得出

3.5.3.模型求解
对线性规划模型用LINGO求解得到:x1:x2:x3:x4:x512:17:18:23:9,分析结果可知,病床数之和为79,使病床使用量达到动态平衡的饱和状态,故所得的结果具有较大的合理性。

4.模型误差和优缺点分析
4.1.误差分析
(1)数据的近似误差
在建立模型以前,为了更容易利用数据,对数据进行了近似处理,必然也存在着一定的误差,而对数值的平均和求解其概率,都是一个近似的问题。
(1)因素考虑的不全面所带来的误差
虽然附件中给出了大量相关数据,但由于病人是随机抽取的,7月13号之前和9月11好之后的数据题目中没有提供,这样就可能存在某些病人的数据缺失的情况,因此我们统计的数据与真实的情况存在一些偏差。
为了统计计算方便,有必要剔出一些不规则数据,即奇异值,且把某个时间点附近测试的情况全部归到该时间点,这样也会造成所建模型与真实情况有一定的差异。 4.2.模型的优点
1、模型在确定医院眼科病床的安排指标体系中,综合考虑了病人的满意率和医院的工作效率等两个方面,其中病人的满意率又细分为病人对从门诊到入院的等待时间和从入院到手术的等待时间的满意程度,并能根据实际情况赋予不同的权值,医院的工作效率联系了国家标准的病床周转次数,所得模型比较合理。
2、模型采用了区间估计的方法,通过对大量数据的分析,具有很高的可信度。
3、可以作为查询服务平台,具有很高的实用性。 4.3.模型的缺点
1、模型在对患者身体状况优劣的研究上,认为所有患者具有相同的体质,接受治疗的效果是相同的,对其他因素没有过多考虑,当然这种情况是无法避免
的。
2、对于病人的住院时间,我们通过统计的方法求得其平均值,在模型中加以使用。但在现实生活中,病人的治疗时间是随机分布的,这样就存在一定的误差。
5.模型的推广
对于建立的模型,我们在将其仿真之后做成可视化的界面形式,提供给医院作为医院门诊病人的资料库,在病人输入患病类型之后,系统能够自动的告知患者前来入院的时间。
本模型利用大量有效、可靠的原始数据和相关信息,利用排队模型,和统计预测的知识,对医院就诊病人数量,病人入院时间安排效果进行分析、预测。得出了患者满意度和医院病床利用率都较合理的预测结果,此结论可直接应用于医院的查询平台系统。另外,模型中分析问题、解决问题时的一些独到方式、方法可以直接应用到银行,火车站等公共设施排队问题的分析上。模型中所引入的一些新的提法(例如:等待因子)等,也是与现实中的情况紧密相关的。通过对其量化,从而使其在很大程度上反映了人的真实心理,使模型的实用价值更强。
